Medical Forms We are mandated by the by the State of Pennsylvania to ensure that all children receive the proper vaccinations and physical and dental examinations prior to enrolling in school. Links to the examination forms can be found below.
Dental screenings are required for students entering Kindergarten, Third, and Seventh grade, or those coming from out of state or country.
Physical screenings are required for students entering Pre-K3, Pre-K4, Kindergarten, Sixth grade, or those coming from out of state or country.

Prescription and Over-The-Counter Medications Students are not allowed to carry any medications, including Advil or Tylenol, in their school bags, pockets, or lunch boxes. This policy ensures the health and safety of all students.
All required prescription and non-prescription medications must be brought to school by a parent in their original containers along with the completed Request for School to Administer Medication Form. It should only be taken by the student in the presence of the school nurse, district nurse, or school administrator.
